DEPOSIT AND CANCELLATION POLICY FOR DENTAL SURGERY
If it becomes necessary for you to cancel your surgery 2 weeks or greater of surgery, a cancellation fee of $500.00 will be charged. If it becomes necessary for you to cancel your surgery 2 weeks or less before your surgery, 20% of the deposit will be forfeited.
